Non-accredited, Non-public School Full Report  - Due September 29 @ 3:00 p.m.

Use this form if:
  • this is your first year home schooling
  • your are returning to home schooling after being enrolled in public school
  • you have a new address
You will also need to provide current vaccination records or a notarized conscientious objection statement

Letter of Intent to Continue to Provide Instruction - Due September 29 @ 3:00 p.m.
Use this form if you home schooled last year and are continuing to home school for the 2017-2018 school year. In addition, if you have a student age 12 (Grade 7) you will need to submit current vaccination records or a notarized conscientious objection statement.

Student Report for Aids to Non Public Students (ED-01650-30) - September 29 @ 3:00 p.m.
Use this form to request reimbursement for home school expenses. If you are expecting reimbursement do not check the nonparticipation boxes on this form. The allotment for textbooks, standardized tests and individualized instructional materials for the 2017-2018 school year is $91.60 for students in grades 1 - 12, and for kindergarten students entitled is $45.80. This amount is subject to change and the allocation amount is provided to us by the state. Any forms received after 3:00 p.m. on September 29 will not be reimbursed. Please note: only expenses for the 2017-2018 school year are eligible for reimbursement. Purchase or order your materials after July 1, 2017. Receipts dated prior to July 1, 2017 will not be processed for the school year 2017/18. You must submit receipts and the items must also meet state guidelines. Deadline for presenting receipts and materials for reimbursement for the school year is April 15, 2018.
 
Please submit this form in addition to the "Student Report for Aids" form if you expect to receive reimbursement for your expenses.
