IF YOU DON’T NORMALLY GET A FLU SHOT THIS SHOULD BE YOUR YEAR!
We accept the following insurances:
- America's PPO
- PrimeWest
- Blue Cross Blue Shield of MN
- Medicare
- South Country Health Care of Alliance
- Health Partners
- Medica
- Preferred One
- United Health Care
This clinic is a MN Vaccines for Children provider and will offer free flu vaccine to uninsured children 6 month to 18 years of age.
If no insurance: $35
Clinic Reminders:
- To save time, complete the Consent Forms in advance, print, and bring to your appointment
- Please wear short sleeves
- Ages 3 years and up may receive the flu vaccination
- REQUIRED: bring your health insurance card or a photocopy of your card
- If paying out-of-pocket make checks payable to: Milaca Public School or correct cash amount
- Come at your scheduled time
- Each family member needs a time slot
- 6 people are scheduled per 15 minute time slot
- If a time disappears 6 people are already scheduled at that time
