Clinic Reminders:
*Please wear short sleeves
*Ages 6 months and up may receive the flu vaccination
*REQUIRED: bring your health insurance card or a photocopy of your card
*If paying out of pocket make checks payable to:
Milaca Public School or correct cash amount
Cost if uninsured over 18 years: $35 injection $40 FluMist
Cost if uninsured 6 months to 18 years: $20
*The 65+ quadrivalent dose is available
