SPECIAL or MODIFIED DIETARY NEEDS
Does your child have an allergy or need a special diet? Are they lactose intolerant?
With increasing allergies and other medical concerns, we need to be aware of special dietary needs for the safety of your child. Notifying us of any special dietary needs your child has ensures that they can still enjoy nutritious and delicious meals while avoiding any potential allergens or foods that could negatively impact their health.

The Special Diet Statement form can be submitted and must be filled out by the parent and a physician according to state requirements.
